OUR PROCESS

Choosing a disability support provider can be intimidating when you’re not sure what to expect. At Great Mates, we’re always open and transparent about the way we work. You’re in good hands.

We make joining Great Mates as easy as possible for you.

HOW IT WORKS

While it’s not always a linear process as every person is unique with different circumstances, we do have processes, policies and procedures in place to make sure you or loved one will be getting the right level of disability support care while you’re with us. There’s a few important steps we’ll take you through when you apply to join our family (please note that timings are provided are an estimate and may vary depending on circumstances). Don’t worry, we’ll be here for you every step of the way.

Step 1

Receiving your form

Time expected: 1 day

We receive a filled out form from you with as much detail as you can give us. You can give us a call on 1300 333 900 so we can fill it out together, or if you’re comfy to do so you can use our online form.

Step 2

Assessing your profile

Time expected: 2 days

We’ll go through your form in detail and be in touch to schedule a meet and greet so we can get to know each other! Depending on how this meeting goes, we may also ask you to fill out our Individual Participant Profile (IPP) which will help us determine the right level of care you need.

Step 3

Getting your ‘yes please’

Time expected: 3 days

Once we have all the information we need from you and you’re happy with Great Mates as your disability service provider, we’ll ask you to sign the Service Agreement. It’s important you ask questions at this stage so you fully understand what you’re signing.

Step 4

Making a plan together

Time expected: 2 weeks

Now to the exciting part! We’ll put together a personalised plan just for you, taking into consideration your abilities, care needs and interests. We’ll touch base with you along the way so you’ll always have your say in the way you want this plan to work for you.

Step 5

Getting started

Time expected: 3 days

We’re almost there! This is an important turning point as you’ll be coming in to meet us to talk next steps. We’ll go through our induction process and everything to expect when you officially join us – the do’s, the don’ts, all the things to do, and so on.

Step 6

Giving us feedback

Time expected: 8 weeks

We really value your thoughts and input as this will help us to grow and continuously improve the disability support care we are providing you and your Great Mates. This will involve answering a short feedback questionnaire and leaving us an honest Google review.

Step 7

Reviewing your plan

Time expected: 24 weeks

We understand priorities may change further down the line and that’s ok. Once you’re settled in, we’ll find time to sit down together and review how things are going. If needed, we’ll make changes to your plan with your approval.

What disability support services does Great Mates offer?

Who needs home and living support?

Like everything on the NDIS, home and living supports are determined by your individual needs and lifestyle. However, you may be eligible for home and living support as part of your NDIS plan if:
  • your home or living arrangement isn’t suitable for your disability-related needs
  • you have restricted or limited mobility and have trouble with day to day activities
  • you need a specialist home and living solution to be able to receive the care that you need
  • your quality of life would be significantly improved by moving to alternative accommodation or by receiving additional support
  • you’re currently in temporary accommodation or somewhere unsafe or unsuitable for your disability-related needs.
